The development of non-steroidal anti-inflammatory drugs (late 1950s) enabled the steroid doses to be lowered and used much more for short courses. Once the child has been on physiological dose of oral hydrocortisone for at least 8 weeks, perform synacthen test. If you have diabetes, you will need increased monitoring of your blood sugar levels while you have the steroids and may need to be admitted to hospital to be offered additional insulin treatment. Taking any drug while using steroids puts extra strain on your liver. As steroids can affect your mood it is advisable to avoid other drugs that change your mental state. Injecting steroids carries its own health risks, such as skin abscesses or infections, and sharing injecting equipment spreads serious infections like HIV and hepatitis C. Steroids are also known as anabolic steroids, ‘roids, gear, sauce or juice (e.g. ‘on the juice’).
Steroid tablets
You may be prescribed steroid tablets if you’re struggling to control your symptoms or if you’ve had an asthma attack. Not all relapses need treatment as, in most cases, the symptoms will gradually improve on their own. If the symptoms of your relapse are causing significant problems, such as affecting your eyesight or making walking difficult, your MS team or GP may suggest that you have a short course of high dose steroids. They should explain the benefits and potential side effects of taking steroids so that you can decide together on the best course of action in your particular situation.

If you notice any changes in your mood or behaviour, tell your doctor or nurse. They may make some changes to your treatment if the side effects are causing you problems. They can be given as tablets or injected or by infusion (a ‘drip’). The absence of cushingoid appearance does not mean that the patient is not at risk of adrenal suppression. However, if a child has cushingoid appearance, he/she should be assumed to have adrenal suppression. The absence of clinical symptoms of adrenal insufficiency does not mean that the patient is not at risk of adrenal suppression.

If you’re worried about your behaviour, or that of a relative, talk to your health team. Keeping a close eye on your mouth can help you spot this early and get it treated by your doctors. You may also want to try changing your diet, to help manage this side-effect.
This means you should avoid any live vaccine until at least 3 months after your course of corticosteroids has finished. If a woman needs to take steroid tablets whilst she is breastfeeding, prednisolone is usually recommended. Steroid injections, inhalers and sprays aren’t thought to pose a risk to babies being breastfed. However, they’re not usually recommended unless the potential benefits outweigh the risks.
Discontinuing steroid (Duration of treatment ≤ 6 months or intermittent treatment eg multiple short courses)
Avoid close contact with people who’ve recently had live vaccines taken by mouth (oral vaccines) such as oral polio or the typhoid vaccine.
